2-Stage Revision Rhinoplasty This patient had a silicone implant that was thinning the skin and nearly protruding. In the first stage, we removed the implant to allow the skin to heal and strengthen. Once stable, we performed a revision rhinoplasty using the patient’s own rib cartilage and DCF (diced cartilage fascia) to reconstruct the nose and restore natural contour.

This patient presented after a prior silicone implant rhinoplasty that resulted in irregular nasal contour, visible implant edges, and retraction, creating an unnatural appearance and imbalance with the rest of her facial features. Over time, the implant contributed to contour distortion and soft-tissue compromise, which is a known risk in implant-based rhinoplasty, particularly in thinner skin patients. She underwent revision rhinoplasty performed by Dr. Donald Yoo, with complete removal of the silicone implant and reconstruction of the nasal framework using rib cartilage and the DCF (diced cartilage fascia) technique. Rib cartilage was chosen for its strength, biocompatibility, and reliability in complex revision cases, allowing for full structural rebuilding rather than replacement of another implant. The DCF technique was used to restore smooth dorsal contour, address retraction, and provide a stable, natural-appearing bridge under compromised soft tissue. In addition to nasal reconstruction, facial balancing principles were applied to ensure the revised nose harmonized with the patient’s overall facial proportions, resulting in a more natural and cohesive appearance. The revision focused on eliminating implant-related irregularities, rebuilding stable support, correcting retraction, and achieving balanced, long-term results using the patient’s own tissue rather than synthetic material.
After two previous rhinoplasties this patient was left with alar retraction, pinched nostrils, and significant nasal obstruction. Restoration revision rhinoplasty with rib cartilage and composite grafts from the ears were used to improve the airway and restore a natural appearance to his nose.
Before and after restoration revision rhinoplasty to create a more refined appearance in a permanent fashion. A previously placed silicone implant was replaced with her own rib cartilage and fascia (DCF) to create a more unified and balanced aesthetic.
